A documented qualification decision, a quality agreement that names deliverables and escalation paths, periodic review of the vendor's performance against defined metrics, and a record of what the sponsor did when performance slipped.
Qualification is a decision, not a questionnaire
The file should show what was assessed, what gaps were found, and why the vendor was approved anyway if gaps remained. A completed questionnaire with no decision record does not demonstrate qualification.
Metrics that mean something
- Monitoring visit timeliness and report turnaround
- Query aging and repeat query themes
- Protocol deviation rate by site and by category
- Safety report submission timelines
- Open action items from previous oversight reviews
Escalation is the part usually missing
Most oversight programs collect metrics. Fewer define the threshold at which the sponsor acts, who acts, and what the action is. Without that, the metrics document a decline rather than prevent one.
